If you have concerns about possible Neurodevelopmental Disorders, e.g.
  • Attention Deficit Hyperactivity Disorder (ADHD)
  • Autism Spectrum Disorder (ASD)
  • Developmental Coordination Disorder (DCD)
  • Foetal Alcohol Syndrome Disorder (FASD)
  In the first instance:

Give parent the NDAS leaflet and suggest that the parent makes contact with the child’s Health Visitor or Early Years/School setting to discuss concerns (i.e. it is more helpful for HV or school to refer, because they hold important information of the child’s functioning in the school setting). As this is a specialist service, there is a recommendation that children will have been through the staged approach and will have reached level 3 of this approach prior to being referred to this service

 If parent still wishes GP to refer:
  • Give parents the NDAS leaflet, an NDAS Information form AND consent form
  • Ask parents to sign the consent and complete the parental information form. Send this to:

  • If parents are in agreement, contact named person for the child to discuss appropriateness of referral at this stage
  • If parents do not wish you to contact named person, contact NDAS coordinator to arrange a discussion with a member of the NDAS clinical team
